Insufficient space in contact name field in HSMP appl form

Hi,

My name in passport is Dhinesh Chakravarthy. I am not able to fill my complete name in contact name field of HSMP application form, due to insufficient space. Similarly, my name on the credit card is also too long, that the payment details Name on the card field doesn't have space to accomodate it. Please advice me how to fill the application in this case. Thanks.

Put two letters in each box
