HMPO won't issue a British passport in a different name to any other uncancelled passport so be careful here.
First and middle name(s)
Please add your first and middle name(s) in the appropriate box. A maximum of 30 characters can be shown on your passport. If your name is too long to fit in the box provided or you have multiple names, then enter as much of the first name as you can followed by the initials of your middle names, in a way that you would want it to be shown on your passport. You can add the full first and middle names in the 'additional information' box which will appear on a later screen. We will add your full name on the observation page on your passport.
If you no longer want your middle name to be included in your new passport when it has been included on previous passports, tell us in the 'additional information' box. You will also have to provide supporting documents that show that you no longer use your middle name.
If you don't tell us to remove a middle name recorded on your birth certificate, previous passport or other supporting document we will include it on your passport.
Surname(s)
Please add your surname in the appropriate box. A maximum of 30 characters can be shown on your passport. If your surname is too long to fit, please shorten it in a way that you would want it to be shown on your passport. You should write your full surname in the 'additional information' box, and we will add your full name on the observation page on your passport.
If you are changing your name, put your new name in the 'Surname' and 'First and middle names' boxes, and put your previous names in the 'Maiden or all previous names boxes'.
Dual Nationals
The name that is shown on the passport should be the name that is used for all purposes. If you're a dual national and hold a non-British passport in a different name, you must change it to match the name you want on your British passport. You must do this before you make your application.
