Tier 1 to ILR questions

Section 12 has this question.

For the period before you were granted leave as a Tier 1 (General) migrant, documents
showing that you met the relevant requirements of the immigration rules.


What docs need to be enclosed ? Does entry clearance visa stamp on passport do ? Note that it mentions "for the period before you were granted Tier1".

NHS surcharge

I am applying for ILR SET(O) Tier 1 General 5 year route. Do I need to pay NHS surcharge ?
Did go through previous posts. but confused.
