Tier2 to ILR Please Help

I am on Tier 2 general Migrant visa and am applying for ILR for me, wife and daughter on 23rd Dec 2016 through premier service at Glasgow.
Me, My wife and daughter got Tier 2 dependent visa entry clearance on 18th Jan 2012 for Tier 2 General Migrant however we came to UK on 1st March 2012 and started work on 2nd March 2012 and am working at the same place. We applied for Tier 2 extension in 2015 and now our Tier extension is valid upto 1st March 2018.
During this time period the following duration me, wife and daughter have been out of UK.

Are we ok in time duration to apply for for ILR on 23rd Dec 2016 for me , wife and daughter, as I have taken in consideration the 28 days prior to qualifying period which I have taken as start form 18/02/2012?

2. On the SET(O) form on section 7.13 at page 28, its asks How long have you lived in UK: I have written 4 year and 9 months. Is that ok ?

3. My wife biomertic card its states : Restricted work, No Dr/Dentist Training.
Is she eligible to work a dentist ? as This Dr/Dentist is training is for Hospital Job right? or even dental practices?
4. Document from employer : states I have spend a continuous period of 5 years in UK. How would you advice this sentence, as I have not spend a continious period of 5 years as actually its 4 years and 9 months and how to justify in the letter from employer ?
