EEA Permanent residency joint application

Hello,

I am trying to apply for a PR for my wife (non-EEA) and myself (EEA) using the form EEA(PR) Version 03/2016. I read through it, I couldn't figure out how to do a joint application, since the documents that are required for both are the same. Do I need to fill in two forms and send them together, or is there any way to do it on one form, or do I need to apply for both separately?

One form.
You as main applicant.
Wife as dependent.
Skip section 2.
