Passports covering the WHOLE 10 years plus what you state above. Form Set(LR), link to form and guidance notes below. She can apply in person or by post. Make sure her absences are within the limit and that no single absence is 180 days or more. She can apply no sooner that WITHIN 28 days of 3rd May, if that is the date she first ENTERED the UK.

Without anyone knowing yet what the fee increase is going to be, and last year was a substantial increase in ILR fees - 25%, perhaps paying the extra £500 to apply in person might be well worth it to get a decision on the day.
