My question is this, do we have to provide the proof of self-employment again when we complete the PR application? What if someone was to lose there paperwork after 5 years of continuous residence in the UK?

It would prove very challenging to make the case for confirmation of PR if vital supporting evidence is missing.
Rule 1 of UK immigration is keep everything - for ever. Period. Oh and keep scans/copies of everything too.
