Tier 2 (General) dependant visa

Hi Everyone
Im planning to apply dependant visa for my wife. Im currently working for a company which provided me Tier-2 General A rated work permit visa. I have been working for this company for 1 year and I still have 4 more years visa.

My salary is ‎£20k per year which was the basic pay for a work permit at that time (2016). But now it has been increased to ‎£25k this year (2017). So will this affect the dependant visa application which I am going to apply ?

Since my visa is a A-rated sponsorship visa do i have to submit the tenancy agreement ? Or just the letter from my sponsor is enough ?

How much will be the cost of the application and NHS fees ?

Documents Needed:

my wife document:
Marriage certificate
tb test
passport copy
graduation certificate
marriage photos

My document:
my attested passport copy and cos
brb card
Maintenance certificate from my sponsor
pay slip 6 months
bank statements 3 months
offer letter
IHS paid reference

Can you please refer these list of documents and let me know if I need to provide more documents ?

Tenancy agreement is not mandatory, a letter from landlord will suffice saying that he is happy to let your dependent stay as long as needed.
The cost of NHS surcharge can be 600 pounds for 3 year visa, and application fee will be around 500 pounds for 3 year visa.
