EEA2 Question — Where do I need to be?

Hi, can someone help with a couple of basic questions? I am UK-national. My non-EU wife has an EEA1 FP (a passport stamp which we got at the airport) — SS route.

What docs do we need to show to get the EEA2 5 yr FP?
Do I need to prove that I live in the UK? Do I need to be here the whole six months?
Do I need to prove that I still live in Spain (where we came from)?
When should she apply? Now? At the end of the six months?
