An Form Help Employment History

Dear All
Once again I need help.
I was employed and now self employed.
As self Employed there are a few questions I need help with:
1.47 Name or Employer or Business?
1.48 Address of Employer or Business?
1.49 Address of Tax Office or Tax Office Reference?
1.50 Employment History. Do I start with the most recent?

Anyone please, I got an appointment at NCS in days time and have not filled it yet.

U said "I was employed and now self employed" & "1.47 Name or Employer or Business?"
Please give a little detail What is it that you do now?

"1.48 Address of Employer or Business?"
Again to correctly answer need to know what you do...

"1.49 Address of Tax Office or Tax Office Reference?"
call your accountant or bookkeeper and he will give you the complete address & or Tax reference

"1.50 Employment History. Do I start with the most recent?"
doesn't matter what order u chose, just don't hide anything, cover complete 10 years Employment history and attach extra sheet if needed

Courier Driver, and when started with them they asked me to provide UTR number. This is the job I am in now. So I remember ringing HMRC to ask for this number.
I will contact HMRC and the person who helps with Self Assessment for more info.
