Tier 2 Dependent visa

Hi,

I am getting end of this month and planning to apply tier 2 dependent visa for my would be wife.

Could someone please help me what documents I need to carry and other details?

I was told that these documents are sufficient

1. Marriage certificate
2. BRP copy
3. employment letter
4. COS number
5. TB certificate
6. NHS Surcharge
7. wedding photos and card
8. bank statement

If I missed any other documents please let me know and could you please help me with the link to application form. I had some hard time finding it.

Is it an arranged marriage?

What about proof of communication?
