I have one full time employee for more than 12 months
And 2 part time more than 12 mounths

How I can fill this section ?

I have confusion about your question.

The titles in the table clearly say Employee Name, start date, etc.

What is it that you have a confusion about ?

Remember this is a table for Jobs, not employees

job 1 full time for more than 1 year
This is clear to me

Job 2: 2 part time employee for more than 1 year


Q: Do I need to mention these 2 employees under 2nd job

You would do the two PT employees as separate jobs - job 2 and job 3.
