under type change (9) tick the "other" box and fill the explination in the box provided below.
About the changes in name, nationality and other things, they said This "can include changes to their:
• Name
• Address
• Telephone number
• E-mail address
• Nationailty"

but it does not say that only THESE can change. So its only set of example they have provided.( these are the common cases for applying change request).

Hey, Thanks for your reply. My confusion is how to send the signature..? is it enough if I put the signature and explanation of it at description in change circumstances form ? or Do I need to send HSMP application form again?

Or can I fax the HSMP application form again?

Please let me know your suggestions on this.

Thanks,
lata.

What I would do in your situation is that I will fax the change request document and a new cover letter authorizing the signature on the change request form to be used for transaction.
They need signature on the form because legally they have to take your consent before using your credit card. Since you have not signed in the original form, you are sending a latter authorization them to use the credit card.
I would not fax the HSMP application form again.
