Which form for Tier 1 FLR (with HSMP approval letter)?

Can anyone please give link to FLR form? I can't seem to find it on BIA website.

I am currently on WP and got HSMP approval letter in Dec 2007.
So I need to apply for FLR under Tier 1 category.

I understand that the fee for this is £350 for postal application.

But how much for personal application?

I believe the document titled "Application for initial entry (switching) or extension of stay within Tier 1 (General) - main applicant" is the one I need to fill.

But seems to be a combined document for tier 1 application as well as FLR.
Can I only apply via post?

It is the same document for HSMP FLR also.....i think you should fill section 3c along with mandatory sections....please read the document it is more clear.

Yes U can apply by post or courier...but post is safe.

Please note, to use this document u should be in UK( I mean in country applications only)

I see that only following pages of 40+ page document are relevant for those already having HSMP approval letter. [ 4-16 & 34-40]

Can I just print and send these pages or I need to send the entire 40+ pages?

The reason - sending extra 20+ pages via special delivery will cost a lot.

Also, the document doesn't looklike fillable on screen. That means I need to print it and fill by hand.
