If it's over cap oversubscription, HO's email should state 'Refused'

The reply from HO is like "Your application has been refused, as it did not meet the minimum points score for the allocation of a restricted CoS on the above decision date."
In November, HO raised request to send documents to support our application which my employer didn't receive until after the cut-off date of 5th December. My employer then sent all the information at the end of December, requested by HO during November allocation.
Now, the HO didn't raise request to send any additional documents this time, but directly refused. Does this mean the HO is satisfied with our RLMT, but refused just because of low score compared to others?
Or they didn't even bother to review our application and refused just because of low score. Does anyone have similar situation like this, where HO first refused due to low score and then asked for additional documents the next month when reapplied?

Yes, my employer applied on 2nd Nov, HO raised queries on 3rd Nov but sent it to wrong email ID which employer never received. Finally got rejected for November allocation and after chasing HO got the queries list on 6th December, just after the cut-off date for December application date. Employer reapplied on 8th December, but was not considered for December allocation and the status was 'Pending' throughout December. Your employer has replied to HO queries around end of December, was that the same got refused now ?
Which means you haven't had refusal in December month, you had it only in January

My application has also been 'refused'!
This means quota for this month is again full and cut-off appears to be somewhere around 55-50. This honestly is quite horrible and considering I applied in Nov 2017 (when cut-off was 21) and refused at the end of Dec is unfair.
