JCAP form questions

Regarding the passport application form -

(1) It asks for parents citizenship and nationality at time of birth -- is it just asking for the citizenship at time of birth? Or two counties?

(2) the NDRS tells us to put our mobile number in S8 rather than in S2. Do we still include our email or is that also S8

(3) where do we put our payment info?
