VISA Fees for dependants

hi

i recently got approved fro Work permit of 12 months , i want to take my family with me i.e. my Wife , 2 Sons (Age : 3yrs and 2 yrs).

what forms i will have to fill in for their application and what fee i will have to pay.
I read on the website that Wife/Child has a fee of 270GBP per head . and the Work permit holder fee is 75 GBP . is that correct .
i need guidance , especially from some one is pakistan .
need help pl.
