Self employed Tax reference number

Hi everyone,
I am filling the AN form and in the Details of your employment, I got a bit confused.
I am an IT Consultant, I work through a limited company am I right assuming :
1.45 What is your occupation --> IT Consultant
1.46 Are you --> A director
1.47 --> Name of my company
1.48 --> Address
1.49 --> My tax reference or my company's ??

please check on any letter issued by HMRC or just call HMRC ....
