Form HSMP1 - how to ?

Please explain - if I dont want (or dont have anything) to fill in some particular sections (e.g. 3,4,5) - should I leave them empty, write N/A or anything else ?

I just put 'Not applicable' for any fields I did not use.

N/A with a line across the section is what I use.
