Does that mean if for example : I am completing 6 years discretionary leave on 26th October 2018, can I ook a premium service appointment from 15th August 2018 (45 days before 29th September 2018) onwards for the appointment to be any day after 29th September 2018 (this date is 28 days before the expiry of discretionary leave)?

7-10 days, as the letter you got would clearly state. Sometimes it is quicker, sometimes it takes longer!
