Tier 2 general visa application -Issued any visa for the UK section

Hello,

There is one section in visa application which states
"Have you been issued any visa for the UK, UK Overseas Territories or Commonwealth Country in the last 10 years?"

I came to UK in Dec 2017 on ICT Longterm dependent visa which was valid till July 2018 . Then I applied for extension and it got extended till Aug 2019 within UK. So now my question is how many entries should I put under this question . Please let me know which Answer should I put

Answer1 : Only one entry
Got ICT dependent visa from Dec 2017 to Aug 2019

Answer2: Two separate entries
1.Got ICT dependent visa from Dec 2017 to July 2018
1.Got ICT dependent visa from Aug 2018 to Aug 2019

Only if you have something to explain/clarify that you can't do in application. Otherwise not mandatory.

I have 1 more query my start date in COS is
11Aug and now i am going for application in october so do i have to mention this in Sponsor note? Is it mandatory? People say its good practice. What's your suggestion on this ?

Also if I don't put what will be my visa dates if not refused?
