Urgent: Please can someone provide a SET(M) Paper form

Hi,

My wife and daughter have a same day appointment for next week for their ILR using SET(M) form.

I had not downloaded the SET(M) form as I had seen they keep updating forms every now and then.
Today when I logged on to download the form I can see that they have switched to online form and there is no option to download the SET(M) form anymore.
I called up UKVI contact centre and they are telling I should have downloaded it before! The other option is to reschedule the appointment which is not ideal.

Can anyone please share SET(M) form - this is the July 2018 version please if anyone has it on their computer?

I am applying for settlement through 5 yr route

For a Premium visa appointment, does it have to be
1) a completed paper form
2) a print out of the online form? I am being asked to pay on the online form, but i already paid £2999 while booking the appointment.

I have already downloaded the form SET(M) and started to fill it up a couple of weeks ago,now looking for a PSC Appointment but nothing.
Can you confirm that I can still use this form?

Paper form should ONLY be used if you already have an appointment with a Premium Visa Center (or secure one until the end of the month)
Others should apply online and use the new UKCAS service instead.

I assume the postal route will still be processed but I advise against it. Apply online, pay the all the fees and get confirmation and biometrics on the spot. It is a no brainer in my opinion
