ILR which form Set(O) or Set(M)

My situation:- I got my 'Tier 1 dependent' visa renewed after my husband received his ILR via the 5-year route on PBS. He has now taken a British passport and I am applying for an ILR.

Request advise on which form to use and what documents to use if making an online application using new service after the 5th of November.
