My ex employer wants to know whom specifically should the absence letter be addressed to.

Currently have:

UK Border Agency
UK

I want to apply at a premium service centre but don't know which one at this time

UK Visa and Immigration Service

When do you plan applying?? The system and service has changed very recently.

Unlikely you will be able to get a old system premium appointment but worth a try.
