If you have lived 19 years legally in UK then might have already secured at least PR/ILR which can get you exempted from paying any IHS.
Your illegally spent time period wont exempt you from IHS rather your existing visa category will determine that whether you need to pay it or not.

Do we know what date it takes effect from?
I don't think it will be beginning of December. It's not like the visa is cheap. Now it's gonna cost more than 2k to apply. Imagine if you have 2-3 kids applying with you, you need a loan to apply. Daylight robbery. Soon it will be EU citizens who will probably have to pay the same.
