Health surcharge, Spouse visa extension

Any assistance would be greatly appreciated. Spouse visa extension on 5 year route. Do we have to pay the NHS surcharge again? Does it matter that my wife is working and already paying national insurance?

Seems like a double penalty, effectively paying twice.

Yes. Note it is also double either later this month or early next year.

No, it makes no difference. National insurance covers more than just NHS.
