The question in form is not asking if u both are applying at the same time at first place but only asks if u are sole director or not which u are not.
I think once u tick u are applying as team, next question is if other member is applying at same time with u and here u can say no and tell the reason.

Cover letter is not mandatory but helps CW to understand ur particular case in detail.

Do I need to create two more jobs after extension or can I use the ones created before extension?
Employee 1 FT(30 hrs a week) started April 2016 and still working(job created before extension)
Employee 2 PT (26 hrs a week) started April 2016 and still working(job created before extension)
Employee 3 PT( 16 hours a week) started October 2017 and still working(job created before extension)
Lastly employee 2 and employee 3 job titles are different. So would there be a problem when I combine both these employees for job creation?

I had extension in December 2017. So I have to attached evidence from that time onwards.
Also how many documents are needed and is it for every month?
Can I print online pdf statements . Would that be enough?
