TB test certificate

Hi All,
Since I'm from India, I need to provide TB test result from a clinic approved by the HO.
I plan to go to Max medcentre in New Delhi.

My question is:
1) What documents do I need to take to the clinic?
2) Do I need to provide any reference # (CoS no, passport no, etc) to the clinic to be included in the certificate?
3) Does the test certificate gets issued to you or it gets sent directly to UK visa centre?

call for appointment to diagnostic centre, then ask your doubt.

As for I remember, you need passport.
They give a certificate and the x ray.

Surprisingly, the girl who took my call at the clinic was quite helpful.

The instruction provided:
Please Bring Original Passport with photocopy
Any Letter issued from embassy
Fasting not required, if wearing glasses carry them
Menstrual Cycle should be completed (Female)
Bring medical history documents
2 Photos required
