Payment details - visa card number

I am going to pay for application by visa credit card.
Should I print three digits of security code printed on the back side of the card in Payment details section?
There is space for 19 digits in card number line. Visa card has 16 digits number.
Are additional spaces for security code or for separation of four digits groups in 16 digits number?

You just need to enter the 16 numbers, after every four digits leave a box blank and the whole number of your VISA card will fill in all the boxes.

Hi Homeboy,

16 digits of VISA card are sufficient, leave rest of the boxes blank. You are never supposed to provide anyone the 4 digits. These 4 digits are used for payment through online (internet). Be careful!.
