Tier 1 Process - do we need to apply for EC seperately

In Old HSMP, we need to apply for HSMP first and once it's approved apply for EC.
Is this changed with Tier 1 ? Does it cover both in one go. Does it mean the 750 GBP we are paying
with the application include the visa fee too.

And do we get the visa stamped on the passport once approved?

Is there any need to apply for EC once Tier 1 is approved.

please help, I am pretty confused.

please clarify

I guess for Tier 1 both the initial application and EC is combined together.

Both in one go (approval + EC).
