It is an entry clearance visa not an extension. Ops wife will get visa valid till the ops expiry date of May 2020.

Pbs extension is for 3 years if the main pbs migrant has ILR.

Any kind of real emergence situation with evidence you can write to home office as exceptional circumstances.
I also had one and wrote them with proof and they immediately expedited my application. After my letter within a week my application was approved.

No i have not attached any personal pay slips with my application. Though I had drawn some salary from the company.
