On the section 4: Parent's details which says

What information do i need to put in there?
Yes, you need a countersignatory.2. Regarding Countersignature. Is that part also needs to be filled in?
Passport and BRP.3. While submitting application, do i need to send my passport as well or just the BRP is enough?

No. The referee must write it as they are certifying it is a true likeness.
