Regarding the bank statements

Could you please help with the following questions?

Is it neccessary for the personalized bank statements to have applicants address on them ?

Is their any format / sample available on bank letters for maintinence funds? Please dont mind if NA.

Also Is it suggestable to enclose a sample covering note mentioning the list of documents provided with the application along with summary in the application?

1. Read para 203-206 of the guidance notes which clearly spell out the details / information that must be reflected in the bank statement(s).
2. Don't think so ... not posted in this forum at least. If you read the para(s) mentioned above, you'll understand what the bank letter must state.
3. Up to you. Personally, I would say it is expected of someone applying under an immigration category meant for highly skilled people. But, just a personal opinion and you might not want to do the same.
