Change in Employer- documents ?-Tier-2 Fees for self and dependents

Hi, If I am provided with new Tier-2 General (Change of employer). What would be my situation with respect to VISA fees and my dependents VISA fees? I have got VISA on Jan19 and have valid Jan 22. Unfortunately, my VISA curtailed recently.
1. Do I need to pay again All the fees including the NHS fee?
2. Do I need to pay again all the fees for my dependents also(Including the NHS fees)?
3. What are the documents required for new employment

Why was your visa curtailed?
1. Do I need to pay again All the fees including the NHS fee?
Yes you have to pay visa fees and IHS again.
2. Do I need to pay again all the fees for my dependents also(Including the NHS fees)?
No.
3. What are the documents required for new employment
Similar to initial visa. The online checklist will tell you. Make sure your sponsor has completed the full and proper RLMT before offering you the job.

I thought your spouse was applying for Tier 2 from home country??
