Surrender Indian Passport

Hi

I have booked an appointment for my child's surrender passport. Question is do both parents need to be there and do i need to take the child too for the appointment?

Next question 2 photos are required. Do we paste one photo in the application form beforehand.

any answers welcome.

Regards
Declaration needs to be signed by both parents, but for actual application submission one parent is enough. Child isnt required. I did this for my daughter last year.
