Tier4 visa application form

Dear all,

I have a question about how to fill Tier4 visa application form. I worked in the UK from 2014 to 2016 and I only went to a clinic once. Now I need to fill the clinic name and address. But I really can't remember the name and address. Anyone know if it will be a problem if I fill the name and address wrongly?

If it was an ordinary visit without any major health issues. it will not be a problem.
