Parents consent forms ??

I am applying for naturalisation for me and my children (born out of UK).
I was told that I will be able to download parents consent forms, but I do not see them on the application page. What should i do? Does anyone have a template? Or probably an empty form downloaded?

There is no template, just write it down in your won words.

I did it and approved.
