Stamp 6 appointment location and documents needed?

I am an Irish citizen through Foreign Birth Registration. I have an Irish passport, but would like to get a stamp 6 in my non-Irish passport to avoid questions when I return from Ireland. I understand that obtaining the stamp is done by appointment. At what address do these take place?

Also, is it sufficient to appear with just my Irish and non-Irish passports? Or should I also bring documents such as my FBR certificate, and the birth, death, and marriage certificates I need for the FBR process?
