She can't apply for it from within the UK. She has to go home first and only lodge the application from there.
The risk of refusal has been evaluated at 99%. This means that it's nearly certain that the application will be refused.
After the refusal, her situation won't be the same as future visitor visas would be declined.

For an adult dependent relative visa, I suggest you read the links below carefully as if you are refused, it shuts the door to any future visitor visas. The ADR is the most difficult visa to get since the rules were toughened in July 2012.
Additionally, for a visitor visa she would have had to prove her ties to home country (social, family, financial etc). Did she lie in her form and present false circumstances??

Most manage to get a visa resulting in a breach of human rights.
It is a sad sad story when ADR cannot be looked after by dependants due to strict rules..the EU make it very simple and cheap to do.
One can only hope rules will relax for both spouse and ADR after brexit.
