this link should take you directly to the page where you can choose the the form that you want. Unfortunately at the moment it does not seem to be working, it could be that they are changing the forms to take into fact that fees have gone up

We should hope they will soon be able to upgrade all the data provided on the web. Since you have time why dont you take some time to detailed guidance for HSMP before doing anything else.
