I have some queries about FLR TIER1(HSMP)Application i.e Judicial Review applicable candidates

1. whether dependant has to just include his/her name in the same application inorder to apply for extension or to fillup separate form

2.In the application it was asked about DAYTIME TELEPHONE NUMBER
THAT MENS IS IT JOB TELEPHONE NUMBER?

3.WHAT IS HSMP REF NUMBER

4.WHAT IS HOME OFFICE REF NUBER?

5.SUPPORTING EVIDENCE FOR EECONOMIC ACTIVITY
whether 12 months pay slips would be sufficient
6.SUPPORTING EVIDENCE FOR MAINTENANCE
whether we have to attach 3 months or 12 months bank statements
7.FOR DEPENDENT RELATIONSHIP PROOF
whether we have to attach Marriage certificate.

whether all of the above documents are sufficient or anything else.
