Documents required for applying Tier 2 General Dependent visa from within UK

Hello,

What are the documents that is required to apply for Tier 2 General Dependent visa from within the UK.

I'm currently in Tier 2 ICT, I'm switching to Tier 2 General dependent to my wife who holds Tier 2 General visa.

I know these are required,

1. Visa details of the main applicant.
2. Passport.
3. Passport size photo.
4. Proof of bank balance for minimum £1500 for the past 3 months.
5. Marriage certificate.
6. Do I need to have any documents from the sponsor?

Am I missing anything here? Do I need to provide TB certificate, I'm here in the UK for 2 years now.
