1. ILR long residence qualifying period starts count from the date you ENTERED the UK and not visa issue date.
2. No, there is no 28+14 days rule. Where did you "hear" this?? You can apply for ilr no sooner than within 28 days before the 10th anniversary of the date you entered the UK. The 14 days rule has nothing to do with this and you are confusing two different things.
