Entry clearance ref number, issue date not known

I have ever lost my previous passport where my first entry clearance visa is. I cannot provide entry clearance reference number, issue date etc. which are required on the application form. In this case what should I do?
Please help!

Say "old passport lost".

If the form asks you to send all (current and old) passports with your application, then you'll have to explain the circumstances in your cover note.
