Naturalisation guide says:
"If you are aged 65 or over or have a long term physical or mental condition that prevents you from meeting the knowledge of language and life in the UK requirement, you may be exempt. You may apply for exemption by indicating this on your application."

The issue is "may". Does it mean that anyone over 65 would get an automatic exemption, or that Home Office would consider it and decide?
If an over 65 person applies without LIUK test, could it create any problems?
The person is still working a demanding job, so it is rather obvious that they would be capable to prepare for the test. However, there is a very small window to apply - next week only - due to them traveling extensively 5 years ago, so no way they can do the test now.

Anyone over 65 is exempt from liuk and English tests as ukvi website clearly states.

Well, I cited what it says (not that one is exempt, but that one "may be" exempt..).
And the application form does not seem to include automatic exemption once age is entered - they filled application, and the checklist still asks about english proficiency proof (?!)...
But it seems plausible.

Another question: they have UK citizen spouse, but want to apply via 5 years route. So they mention it in cover letter. Any other place one can state it?
Again, one has to say that they are married, then the form asks whether spouse is UK citizen, and then the checklist asks for marriage certificate, and passport + naturalisation certificate of the spouse.
Should those documents be submitted, given that they are on check list, even though the person wants to use 5 years rule?
