Tier 2 dependent IHS charge - can I pay it before getting my own visa?

I am applying for a tier 2 (general) visa, and have got to the point where I'm supposed to pay the IHS charge.
Despite explicitly listing my spouse in the visa application, and marking that she will be traveling with you to the UK, the IHS form shows that "You don't have any dependents".

Is that OK? should I have done anything differently?

On my spouse's form, it asks for the "Visa expiry date".

Does it mean that we can only pay my spouse's surcharge while my own visa is approved and I get an expiry date?

Put the end date of your cos duration, or if a 3 year visa, use the end date.
