Will a separate reimbursement slips issued by the company for all the 12 months apart from the main payslip be accepted for calculating the previous earnings.

See Tier 1 policy guidance, points 108 and 109. As per my understanding, any allowances must be present on your salary slips for being eligible to be included in earnings. Still you can confirm this by emailing to local BHC and explaining your situation.

Depends on what these re-imbursements are for?
1. The reimbursements must be for allowance(s) that form part of your remuneration package and included in your payslips (e.g. - medical allowance, housing allowance, transport allowance etc. etc.).
2. Reimbursements for expenses incurred by the employee cannot be included in previous earnings (e.g. - taxi, corporate meals, client entertainment, etc. etc.).

As advised, above, read para 108 carefully. Also read para 107 regarding "allowances".
