FLR (HSMP) Tier 1 Clarifications

I have to apply for visa extension. Initially I got HSMP visa in Jan 2005 and later got extension for another 3years which is about to expire after 3 months. I need to apply for further extension.Please could someone provide the below information

Do I need to apply for visa via FLR(HSMP) TIER 1?
What documents are required as I'm director of limited company?
Can I apply dependents on the same application form?

1) You can download the application form from ukba website the link is

2) You can apply for dependants on the same form included in the fees
