Help with EU1 Form

Help with EU1 Form

In the EU1 Form they have these questions and we got a bit confused.
Date of arrival in the State: day/month/year
Place of arrival in the State:

Date of first Registration in Ireland: day/month/year
If no registration date has been provided please explain why:

Status on arrival in the state (please tick)
Asylum Work Permit/Visa/Authorisation Student Visitor Other

Other (Please give details):
For what I understood
Date of arrival in the State: when we arrived in the State

Date of first Registration in Ireland: the very first time my husband entered in Ireland

Status on arrival in the state (please tick): what kinda of VISA my husband had when he first arrived in Ireland.

Is that correct? Could someone throw some light over this questions?

Another issue is that they ask as original documents for Medical evidence (if applicable), what does that mean?

Finally, there's a INIS's request (already discussed here in the forum) of a residence in Ireland of 3 months before submitting the form, my husband has some bank statements from 6 months ago when he was under a student visa, can we submit that?

we filled it according to below comments, and it was approved. I remember wondering about some ambiguities as well... Good luck!

What about the initial registration?

Do we have to pay any fees or are we free of charge?

What a huge surprise! We came up to the GNIB, we had to wait for about one hour to talk with the officer, which was really really nice (making me wish that all people in government services were like him). Our documents were in French and were not translated but as the officer could speak French he didn't hassle us with translation or anything, very straightforward. My husband took a picture and was fingerprinted and after a few minutes waiting the card was ready with a huge surprise: Stamp 4. We are very happy about the services at GNIB. I hope all the next applicants will have the same treatment as we had.
