MN1 parent consent query

Hello,
I have a minor questions please. I am applying for BC my daughter (2.5 yro) - MN1 application. After both my wife and I signed the Parent Consent (that I downloaded after the application submitted), I've noticed that the Checklist, that was created after application submitted, says about Parent Consent "The letter must: give their full names and addresses say that they agree to let you apply for a British citizenship".
Now, in the Consent Form, there is no box or any mention of address of parents. Shall I enter the address in any space there? Or is the full address ONLY for the Guardian (which is not applicable in our case, as my wife and I are consenting).

Can you take a screen shot of the consent form hiding all personal details and attach here?

I am not sure which consent form the form is generating. As of my knowledge, you have to draft your own.

Template for you, print, fill, sign and upload. The one I used for my daughter.


Consent to the application

Mother’s consent:

I (Full name of mother) consent to this application for the registration of (Full name of child) as a British Citizen.

Address:
Mobile:
Email:

Father’s consent:

I (Full name of father) consent to this application for the registration of (Full name of child) as a British Citizen.

The form is exactly the same as the paper application which can be found from this link. It is Section 7, pages 27-28.

You can simply provide the addresses on the cover letter if not already mentioned in the online form itself.

There is enough space beside signature box so you could write your address there I guess. I'll do the same as I noticed the same in the downloaded consent form. If not, we can use the template @Cullinan gave previously.
My advice is based on personal research/experience, so pls check relevant UKVI sources.

Now waiting for the far appointment on 15/12 :evil: Paid £69.99 for an appointment that is 28 days ahead and I booked it minutes past midnight! How awful those lot!
